Case Summary:
The patient presented with a compromised tooth #15 (upper left second molar) showing the following clinical challenges:
- Long and severely curved canals
- Pulp necrosis
- A long-standing sinus tract, indicating chronic periapical infection
Accessing the canals was particularly demanding due to the patient’s extremely small mouth opening and limited accessibility.
Diagnostic Approach:
Pre-operative evaluation included 3D CBCT imaging, revealing:
- Pronounced curvature in the mesiobuccal and palatal canals
- Difficulty in locating the MB2 canal
- A palatal root apical foramen located short of the radiographic apex, emphasizing the necessity of apex locator verification and 3D imaging for accurate working length determination
Treatment Steps
- Canal Negotiation and Glide Path Establishment:
Initial negotiation was performed using manual files to secure access. Following this, Flexivo Dental EZ-Shaper Files were employed to create a safe and efficient glide path. The EZ-Shaper’s controlled memory NiTi alloy provided exceptional flexibility to navigate through the challenging curvatures without canal transportation.
- Cleaning and Disinfection:
The canals were activated and thoroughly cleaned using the GentleWave® System, ensuring deep penetration of irrigants and perfect decontamination.
- Follow-Up Plan:
The patient will undergo clinical and radiographic follow-up to monitor:
- Healing of the sinus tract
- Regeneration of periapical bone
